Yes, it's almost a certainty Genjutsu was involved.It most likely was a genjutsu.
If it was Izanagi then Madara would have left unscathed but he didn't, also Izanagi requires both Senju+Uchiha DNA to use which Madara still didn't have during that time, about the sword, it was seen the moment Madara fell to the ground and immediately vanished after Hashirama fell on his knees, the interval of which was only seconds so it's impossible that Hashirama had pulled the sword already in the following panel.Very nice additions, as to be expected. Your theory is really beginning to take a form so much so that makes it hard to argue against lol. Every time I critic a theory I always like to find something disagreeable with it in order to promote additional thinking but I think you've pretty much got it solid. Only thing I can do now is nit pick x__x lol

If his eye did make the [fwip] sound after getting stabbed, then doesnt that point more towards the usage of Izangi in order to keep himself alive rather than, my original theory that he used Izanami, or yours which is his use of tsukuyomi (As if it was tsukuyomi then Madara's original body would show the [fwip] and not his fake genjutsu body that only exist within Hashirama's mind).
It is also possible that the sword was missing because Hashirama removed it, he really has no business leaving his sword in Madara's dead body x__x
But yes given the sparkle effect use and the fwip there is definantly some funny business going on that involves genjutsu.

Hashirama was low on chakra, thus the vulnerability.Doubt it, Hashirama fought the Uchiha all his life and has fought Madara before. If Madara could use genjutsu against Hashirama successfully he would have beaten him a long time ago.
But why did he look upward? Madara was laying on the ground, if it was exhaustion, then the instinctual action would be to bow down since looking upward exerts more energy going against gravity's pull.I think the diamond sparkle effect is only used when they realize something..

He's not the sharpest shed in the tool you know, despite his strength. He was certain he had killed Madara, if he fell for a genjutsu and never once began to doubt it that would mean he was a complete moron.

Madara didnt leave unscathed? I know it says that he barely survived the battle but what makes you think he didnt leave unscathed.
