really good list but I bolded a few things I disagree with:
-I didn't find base Naruto, but I would definitely put him in high jonin, early part 1 Naruto mid jonin, I'm not sure where CE Naruto would fit but I think he deserves to be in the same slot as CE Sasuke.
-Haku at low jonin(I couldn't find him)
-Couldn't find Suigetsu or Jugo, but they would fit in the mid jonin, Jugo may be even be high jonin material if he goes full berserk

My only real issue is that some of your versions seem superfluous, I see no major difference between:
-Early part 1 and late part 1 Kakashi doesn't really seem all that different, what makes it weirder is that early Kakashi is a tier lower than Zabuza despite beating him. Also I feel that 3T Kakashi at low-mid kage is a stretch, the only reason he can even compete with the rest of the kage tier is due to Kamui, high jonin/low kage at most.
-Neji and Lee don't really seem to need different forms for CE and SRA. SRA Lee was basically Lee just with a handicap and SRA Neji seems the same as Lee in the chunin exams. Considering they were a older and stronger, it would make sense that they were such a tier higher than the rest of the k11 early on. On that note, I don't like the idea of separating Lee and Guy from their gates since that's what his whole fighting style revolves around, it'd be like taking akamaru away from Kiba imo.
-Asuma a tier below Neji, that's just mean. He should be high jonin, while part 2 Neji and Lee are mid jonin imo
-low/mid kage and low kage seem like they could be condensed into one tier.
-current Choji is pretty damn strong, mid jonin at least, not sure if he was on there.
