It's sound. You need to hear the sound as it is in order for it to work. :| A scream/shout would defend her from hearing it at all or at least make the sound she hears different, thus it wouldn't work.
By chunin level I meant Sakon/Ukon. Kimimaro is a high jonin-low kage. Tsunade on the other hand is high kage level by fighting capabilities alone.

Actually she does. The brain is not important. People in real life have survived objects to the head all the time so someone like Tsunade can easily do it.
