You're overestimating Tsunade. Just because she's a kage doesn't mean she's on the same level as the previous 4. Even at 100%, Katsuya's usefulness is not likely a match for PS. Tsunade is well known to be the weakest Hokage and far inferior to Madara.

Katsuyu calls Tsunada "sama", because it is a japanese honorific, and Katsuyu has a very obedient personality. Don't forget, Indra and Ashura likely have roughly 100x the chakra of a regular shinobi. This means that Tsunade, even with a sage mode, would likely have 50x less chakra than regular living Madara. The Perfect Stabilized Susanoo is on par with the wood golem Hashirama used, and is easily as large as a tailed beast. It took Hashirama using a jutsu far above anything Tsunade could dream of doing to completely crush that susanoo with the kyuubi inside it. For her to beat something of that caliber would likely be near impossible, even if she really did have a slug sage mode already.

Remember who Tsunade symbolize in Japanese mythology? it's the Slug Magic the one defeat Snake and co. it beyond Kaguya story, Indra Ashura or Hagoromo Hamura because in different famous mythology. Everything in the NV storyline is based from The Great Gallant Jiraiya Book. While Madara Indra story is pale comparison to this slug>snake>toad>Slug>snake/dragon>Rabbit. IF all legendary Sannin really have sage mode that means it belong to sage group together Sage of Sixth Path, Great Sage Toad, Dragon Sage or Slug Forest Sage. Madara Indra have no place in this triangle. Sage Mode is what Madara desire for after snatch it from Hashirama the power Madara have is increase alot. Slug sage mode it maybe in same card as Hashirama.
