Tsunade lost because she is a woman. No females in Naruto are truly strong and can win against powerful opponents, at least solo (minus Kaguya, doesn't count). They are all largely weak and useless.

Tsuande is weaker than Madara.
Even if Tsunade summons full Katsuyu , Madara's PS Susanoo will protect him from Katsuyu's acid attacks. 1 attack of PS Susanoo slashes a mountain so it can slash Katsuyu really fast.
Talking about Madara's taijutsu & physical strength, he was able to give tough fight to Hashirama so same can be done with Tsunade.
In most of the battle, he was judging strength of 5 Kages. He can create a lot of susanoo clones which can overwhelm Tsunade.

Tsunade needs at-least 1-2 really hax jutsus & sage mode to fight Madara.

Clearly out of 3 sannins, only Jiraiya had Sage Mode, other 2 were not able to use it so it was never shown up for Oro & Tsunade.

Thats why this time kishi will give her a chance to have noble death in part3. In all previous fight Nagato and Madara all she did is taking care for everyone injuries. Remember when she smackdown both Kabuto and Orochimaru? she did it once. But with Sasuke wont be the same because sasuke no longer snake but will turning to a dragon. Slug>Snake but Dragon>Slug.
