This is one of the sweetest moments in the entire Naruto anime. Itachi is just a 5 year old, running to safety as the Kyuubi is attacking everyone. He feels anxiety as well. But he doesn't care, because all that matters to him at the moment is keeping his little infant brother, Sasuke, safe who he was left in charge of when he himself is a child as well and there's no one attempting to comfort him. <3

I don't think he ever genuinely smiled after he left Konoha to become a missing-nin (I mean happily, he smirked a few times but other than that nothing until his moments in death and when he came back as an Edo about to leave a 2nd time).
