You form some reason mentioned that Suigetsu is pretty strong when he wants to be. But all of that pales by and large to Tsunade's strength. It's not even worth making a comparison in fact.

The problem I have however with you suigetsu catching Tsunade off guard, I'm not saying he can't get a lucky strike. But it would be easier for Tsunade to distract him with a giant slug the size of a tailed beast (which can divide into thousands of minis) and get a strike that way
And yes that was a poison attack when his body reforms the poison would begin to act. But my more pressing point is that Tsunade is a poison user, who taught Sakura, the use of poisons and was able to cure the greatest poisons in the 2nd shinobi war and as she is still a better specialist with poison could probably cure Kankuro's poisons. Which speak of the great knowledge on poisons. What that image proves is 1. Tsunade uses poisons and 2. Tsunade keeps poisons on her person. Given both Shizune's and Sakura's affinity to poison it is good chance that their master too is a poison user.
