I think it is a tattoo. After looking at it more closely. The "eye lines" that look like () sideways don't close. And there is no pupil. So as to it being real, I don't think so. I think it is a reference to an "all seeing eye" of the Uchiha Clan. But good spot

The eye seems to be the same as the one that open on up on Kidōmaru when he went curse mark two. I think it is an eye and not some type of tatoo and/or paint...
