How do you think he'd fare against a (serious) Itachi and why?

Itachi could hold his ground vs an even more advanced sage mode
used by Kabuto, purposely let himself get hit, and won.

Itachi wins, the sword too OP, and I don't believe SM Naruto has
any feats against genjustu, which is Itachi's specialty.

He did cut up a lot against Kabuto.

Those kind of injuries he couldn't afford to sustain in life. And this takes on a similar start to Naruto vs. Pein, with toads and all.

H ewould of stomped naruto but,he wasn't beating kabuto without izanami.

The white rage along with his other justu shut down the sharingn completely.

Only izanami and izanami alone was getting them out of that battle.

Yata has little going for it against non-nature based attacks except defending against a small string of paper tags. Yata's real defence is specifically against the basic natures, changing its own nature to the weakness of the incoming attack; shame a Senpō Chō Odama Rasengan doesn't have a nature.

Besides, Itachi only uses more basic forms of Susano'o when it's necessary. Ever seen him use V4 at the start of any battle? Nope. He hasn't got the chakra to be doing that... Not unless he wants to keel over.

Still doesn't use Susano'o unless he's pushed to do so. And Ma or Pa can break him out.

Itachi has a natural disadvantage of being a progressive fighter where Naruto is all over the shop. What happens to Itachi if Naruto starts going V1, or even V2?

If Itachi catches Naruto in Tsukuyomi, then it is over for Naruto is an instant. If Naruto avoids eye contact, Genjutsu won't work but this will be an excellent opportunity to Amaterasu Naruto. So I don't see Naruto escaping Itachi in anyway. And all of Naruto's Toad Summons will get Totsuka'd. And all his Rasenshurikens will get Yata'd.

Itachi takes this hands down mid diff, TBH.

Was an edo and had Sasukes help gtfoh and izanami gets negged by someone like naruto.
I see this fight being high diff for both with Itachi winning 6/10 if they fought ten times because of genjutsu. However naruto can close his eyes and fight while in sm

How does avoiding eye contact negate him avoiding ama
Nobody in this series avoiding it because of their eyes they did either because of sensory or enhanced reflexes(Ay he did use his eyes but his lightning armor is why he was able to react his bodily functions were sped up). Naruto in sm is a sensor who'll sense ama regardless of What he sees. All don't get totsukad cause susanoo is draining and itachi has stamina problems. Yata is a shield that covers susanoo from one side shadow clones surround you. If Danzos win jutsu almost split susanoos spine than a rasenshuriken is doing damage. If a senjutsy rasenshuriken lands than susanoo gets Pretty messed up.
Now if Naruto does what he did to kurama with his rasenshurikens than you know that small susanoo is getting obliterated. Itachi is a threat to naruto because he isn't reliant on his dojutsut and uses it at the right moment. Itachi takes this high diff because naruto falls out of sm but if itachi is worn by then narutos reserves would really help. Since itachi would know sms weakness in no time he'd save his physically taxing dojutsu like ama and tsukyomi when naruto leaves sm. But Itachi would have to out last sm.
Itachi and Kisame treated j man like a huge threat so sm naruto isn't getting mid diffed by itachi alone

Even with frog song itachi has shown that he can do genjutsu reversal so it's nearly impossible for naruto to win unless he uses kurama
