Theory/Observation: Orochimaru's quest for immortality coincides gently with his endgame which has been the same through the series. Remain immortal and undefeated, with his 'Professor' goal he inherited from Hiruzen to master every jutsu and become the ultimate lifeform, except unlike his predecessor- Orochimaru knows the key to knowledge...perseverance.
But why DOES Orochimaru want eternal life? He's not like Tsunade, obsessed with vanity or Onoki, desperately holding onto life as a cliche coffin-dodger..Oro's intent has always been ambiguous and changing. Or maybe- it's never changed at all.
Think about Oro's point of view. He is an old immortal, almost 100 by the time of Gaiden and keeps progressively getting younger. His motive about 'watching Sasuke's wind' and the guise of being peaceful with Konoha could easily be just...a temporary solution.
He is aware that he could be stamped out in this time era with actual gods watching over Konoha & dealing with the last of the Ootsutsuki. Just like Orochimaru let Team 7 deal with the Akatsuki for him, Oro seems to be doing the same with them and the Ootsutuski- not getting involved in any interstellar threats when he clearly could've been of help for his planet if it were a true alliance.
And so...even though he is weaker, Naruto and Sasuke can't conquer old age. And Orochimaru's intent may be to simply outlast them both. Like a true snake, it's a natural survival/hunting instinct of larger snakes. And a well-known fact that you can't out-wait a snake.
This is instantly scary and reaffirms Orochimaru as a constant threat of this world. Just because he seems placcid and not battle-ready in his current form says nothing about the ominous nature of keeping him in the world. How does Mitsuki play into this? A peace offering between Konoha and the Sound? An unconscious spy into keeping an eye on the next generation. Just in case they turn out to be nearly as strong. Look at the expression below, it's not his usual sly smile like he should have in this scene. Is that concern for a Uchiha predecessor? A possible future enemy.
