Yes, I do have several questions in my head right now.
First, regarding the second bio, I don't really understand why we need a second bio, does it brings any advantage in the RP?
Another question is how can we obtain Genkai/Hidden Ability/Advanced Element in the bio? Does it have any special requirements?
Second, regarding of the skills and combination.
Honestly, I'm kind of confuse but I tried to arrange something out of it.
1. My current bio is a custom bio right? Which mean that I can have either one advance ninjutsu and one mode (based on the chart) , so how do we obtain it? Do I need any special requirements for it? And does that mean this bio is not able to obtain the other category of skill?
2. As for the Genkai/Hidden Ability/Advanced Element skill, do we need to be in a specific clan or get a specific bio in order to obtain certain technique?
3. Most importantly, since I want my bio to be a medical student, any thing for me to take note in terms of skill combination?
Please forgive me if some of my questions looks silly and thanks in advance.
- The idea of a second bio or a third bio is to diversify, try new things and test new waters. Of course you can decide to stick to a single bio. I know of someone who stick to just a single bio and a single element and he did just fine. Multiple bios are especially good if you want to run more than one official battles in the Battle Arena. Of course only one of those bio can be USED in the NW.
- You can add KG/HA into a bio in two ways:
1. Cannon Bio: if the cannon bio is said to have the kg or ha or any ability at all then you can add it automatically. However you need to read up on the cannon charater in the link below to know if you need to buy that character or you can just make them freely

2. Custom Bio: You can add any KG or HA to a custom bio but you need to follow the bio combination rules I told you earlier. Custom chakra are not listed but you can create yours.
Exception abilities that are allowed to be added to both custom and cannon bios are the Universal abilities like the leg weights, Y/Y, NB Tai etc. Also can be found in the link above. You can add any of them to your bio so far you buy them.
Summary:
KG/HA some are free and some must be bought. Some come auto with a cannon character that come free or can be bought. Remember the combination rules for custom bio should you want to have a kg or HA in them. Universal abilities can be added to any of the bios without restrictions.
1. Checked your bio and it's an open one. You can have either
1 HA + 1 AN + 1 Mode
1 AE + 1 AN + 1 Mode(ex SM)
1 KG + 1AN + 1 Mode(ex EIG)
You can check it and every HA, AE and KG there is via their link. Follow the link I provided above and click on each of their name and you will be directed to their Thread. Some of them can be made freely and some requires you to be of a particular rank. Byakugan for one is an ability you can make at your level and is also a KG.
There is another possible combination. They are called Custom clan biographies .
To make a custom clan biography, meet with a clan leader and ask for permission to join their clan. Then you can make a biography for that clan.

Lastly, you can combine two abilities. This combinations are called dual abilities. Eg:
KG + HA
KG + AE
AE + HA

2. No you don't. So far the bio you are making is a custom bio, just go for it while following their individual restrictions which can be found by clicking on them and reading up them in the link above. Only exception is if you are going the cannon route then you follow the restriction of the character itself.
3. Any bio can become a medical ninja. Even cannon bios that are not shown to have medical ninjutsu. Medical ninja is a universal ability. All universal abilities can be added to a bio, so far you already gained them either by winning, buying or through lottery.
