I would still say Tsunade does.
1. Experience: Tsunade's combat experience and career as a shinobi FAR outclasses Neji's. Additionally, growing up in Konoha would make her knowledgeable of the Hyuga style, and have thought of ways to combat it. Just look at Kakuzu vs Kakashi and Team 10. His experience resulted in Kakashi on the ground twice, and twice in positions where he was dead if not for outside help saving him. Experience always plays a significant factor in fights.
2. Strength: In terms of physical strength, Tsunade has one of the highest physical strengths in the series. Even without enhancing her strikes she can crush walls and the earth. So even if her points end up being blocked, she can still dish out the physical damage to hurt Neji if he decides to try and block a punch.
3. Ninjutsu: While in a later phase of the fight making it useless...due to the gentle fist... the early stages can call for her to utilize techniques like the one she used on Kabuto where she re-wired the nervous system to make different body parts move different things. Had she been going for a kill, Kabuto would have been done at that short interval from when he was unable to figure it out.
It is also important to keep in mind that Tsunade is not a pushover when it comes to hand to hand combat. Sure she doesn't have the most elegant style, but if it gets up close, one grapple or one punch that should have been dodged instead of blocked is most likely game over.
Even more so, Tsunade can keep her distance and use the power of her strikes to disrupt and effect the ground around Neji.
I'm glad you respect and understand the power of the Gentle fist; however, between her combat strength and her own natural physical strength without enhancements... I don't see Neji winning. The fact we eliminated a summon for Tsunade should be enough to show that she is stronger.

He has a 4.5/5 for speed while tsunade only has 3.5/5. In these panels, it is stated that the Byakugan is able to sense & detect enemy attacks & movements, which would allow for Neji to be able to know where Tsunade is and what they're doing. He would also be able to detect and sense when Tsunade is getting ready for an attack.
Neji is also fast , not on his feet, but when he is attacking. Someone like Ay is fast on their feet, but Neji is fast when attacking, as evident by the following scan
Neji could attack an enemy 64 times in a near instant [This is true for all users of the 64 palms] So neji's arm-speed is fast. He could very well attack Tsunade fast enough before she'd have a chance to hit him.
Neji has also displayed impressive reflexes. which, as mentioned before, are mostly thanks to his Byakugan.

Combat speed and foot speed are not the same. Neji's combat speed is faster than Tsunade's, which means Neji has the upper hand in a hand-to-hand battle
The Byakugan is also able to keep up with high speed movement. This is apparent by Neji's ability to analyze Kidomaru's high-speed attacks while they are in motion.

Neji was also able to, along with Kakashi, to be able to keep up with Lee's movements and attacks when he had opened the chakra gates. Kakashi did this by using the Sharingan's prediction ability and Neji did it by using the Byakugan's ability to process fast movement accurately.
So the Byakugan is capable of being able to see some pretty fast jutsus, which would allow Neji to be able to keep track of Sakura/Tsunade as well.

So in conclusion, Neji isn't gonna get over whelmed by speed, due to the Byakugan's ability to keep up with high speed movement, detect & sense attacks, and to increase the reflexes of it's user.
With the gentle fist & fast movements, It's possible for him to very well one-shot Tsunade as a simple poke to the heart area, which would damage the heart, would render her dead, or at the least, severly injured and unable to fight ....if it was his intent to kill. When Neji attacked the Tenketsu near Hinata's heart in their fight, even after medical attention AND a whole month to recover, she still wasn't at full strength.
And he wasn't trying to kill her.

Also, Tsunade's strong body is pretty much worthless in this fight, as it has been stated that, unlike the strong fist which focuses on damaging the outer body, the gentle fist attacks the inner body, and that part of the body can't be trained to become stronger. Whether weak or muscular, the gentle fist will have the same affect on everyone.

Heavenly Rotation is also a suitable defense against the Medic's punches, as her punches wouldn't be able to make it through Rotation. The only time anything made it through the Rotation was The Juubi's Spears, and that was due to the speed.....The Byakugan would be able to keep up with speed, thus if Tsunade is about to charge in with the intent to attack, Neji could put up the Rotation technique. There's also the Byakugan's 359 degree vision, which would mean no sneak attacks from behind would work, and would be countered by either Rotation or One blow body.

Tsunade also can't get near Neji because they'd lose the instant Neji manages to land one blow, as he'll easily be able to continue on with more from there.
Also, Neji is able to alter enemy attacks with his chakra. Meaning he is in control of where he wants the attack to go. Somwhat, anyways.

This would allow Neji to either make Tsunade's attacks miss all together, or, if she is possibly too fast for it, he could at the very least slow the attack down to lessen the impact and alter it's direction so that the attack isn't a clean punch, which would save him from suffering too much damage. Also, aside from Heavenly Rotation, Neji has another defense, this being a sort of chakra armor.

This ability, as shown in the image, deflects punches if they were to make it to Neji. Now, obviously, the attacks are a lot stronger than Naruto's, so this chakra armor would do nothing more than lessen the impact.
Tsunade's slug summoning
To put it simply, this thing is essentially pointless. Katsuyu's only offenseive ability is her ability to shoot acid out of her mouth. Sure, a normal shinobi wouldn't see this coming, but Neji, with his Byakugan, could and would use his heavenly rotation as a counter, or simply dodge it all together.
Katsuyu's( & Tsunade's) healing abilities are relatively pointless in this fight as, like I said before, gentle fist affects the inner and not the outer body. Sure, they may be able to heal a few wounds if possible, but would that help reopen chakra points? Nope. and that's another thing, with their tenketsu closed, Tsunade would neither be able to heal or punch.
Katsuyu also could easily be taken out by Neji pretty easily. Since Katsuyu has no arms, she can't do anything but sit there while Neji attacks her. If Tsunade tries to approuch from behind, which Neji will be able to see, the one blow body or Rotation will be able to deal with it.
