Well, this one isn't so much of a mistake(?) as it is more likely someone being lazy:

Kakashi's headband has the bottom half of the kanji for shinobi, which is actually kokoro 心 instead of the full kanji 忍.

I wouldn't have posted this but upon reading it a second time the top of his headband is in fact drawn with the little circles at the top of the box.. thus laziness
