He died of old age. He was the strongest shinobi besides the so6p i dont think anyone killed him.
I think he died when he battled with Kakuzu, as Kakashi said in episode 89 (as for Naruto Shippuuden) with title "Naruto's Injury". Here's the screenshoot :

Definitely not. He'd have to be weakened before then for that. Kakuzu would have been famous and would have been recognized by all shinobi at first sight. Either he had a heart attack induced by Light Yagami, or he was killed in his sleep.
oh and I forgot, one of his heart (Kakuzu has 5 hearts) was Hashirama's heart. I'm sure he was the one who killed Shodai. I've watched it somewhere I forgot which episode was it.
It's a question I've asked myself a lot as well. Edo Tensei appears to revive you more or less at the age you were when you died, given how Tobirama and Hashirama both appear to be in their 30s while Hiruzen still looks like a 70 year old. So he didn't die of old age.
I think the best bet is sickness. Given his feats, and the hype he had during his time ("Strongest Shinobi Of His Time", "God of Shinobi"), I doubt anyone was powerful enough to kill him. He could have probably solo'd the other Hidden Villages if he'd wanted to.
