IMO, the only bad placement is Itachi, and as you said, it was because of the original stock...The plaement is bad on the avatars.I guees it's because of the original image's size.You should either low the canvas size of avatar to the right proportion so the main part could fit or choose another image.
Other than that,nothing much.Try to find more imteresting stocks,that at least have some potential to work with.

The focal point isn't necessarily meant to be on the face(front), but can instead be from the sides(Sannin Ava)...
Anyway, I think it's a matter of "perspective", everyone has his own idea/opinion about Designin(Avatars in this case.)

The really bad thing about avatars is their limited size,so the focal is important and it's not a big deal to figure it up, but my guess is that not every part can be used as focal point,especially in stocks with so little potential.
Underline: true,however from what i see here it's hardly can be called a "Design" .
