How Rin did die?

I was wondering how Kakashi's precious friend Rin died since the Pein Arc because we know that he promised Obito to protect her and after Pein killed Kakashi he saw Obito,Rin and after his father that means they are all dead right?
And also Kakashi stated that he failed to protect everyone he loved before.I know this has nothing to do in storyline but I'm Kakashi fan so i am curious about his past.

it was assumed that she had died since kakashi said that all those who are closed to him had died.. and as i recall if im not mistaken... her soul was seen when kakashi died (temporarily) in Pein's hand.
