We're talking about Hidan not tsunade so we dont have to take her into account. His head needed to be fixed onto his body for him to move again BUT how can his brain work when he got beheaded? A beheaded man's brain would be dead but hidan still was alive.
As you said, it was spiritual. he experimented in jashinism, but from the prospect that he needed to connect his head to his body suggests that nerves are involved, as he'd need his brain to send signals to other body parts in order for movement to be possible. Which we can then further assume that BPD would work on him.
