It wouldn't have returned back to being just a cave if NE inside it was still intact. Kabuto didn't cancel it, but Amaterasu forced it to go back to its previous state; a cave without a life-force or a cave without NE. The spikes literally died.
They couldn't move, because the NE inside it was gone. They couldn't move because NE was the only thing that was allowing Kabuto to control the spike movement. Without it, the cave was no longer animate or alive.

They haven't. That's chakra kneading. Even FTG has this small lag. In fact, KCM Minato was kneading chakra to travel to the aid of Naruto and Sasuke and he couldn't complete it because Obito kicked him and his clone away. Guess what happened? He couldn't get there and use FTG.
Prep and technique being instant is not one and the same as all of them little prep time, and by the way, Raikage dodged behind the flame; it was already made. You also forgot to mention that JJ Obito has Sage of the Six paths senjutsu that is the highest form of Senjutsu known to man. He saw everything coming bar the FTG swap technique because he had no clue what they were about to use.
