If Ay went for the blitz he certainly wouldn't miss. And there's no reason for him to start like that, i don't know why you assume he would.

For Itachi to do the exact same as Madara, first Ay would need to (unnecessarily) be distracted about an Hokage who would (arguably) be doing better than him, occupied with four other Susanoo, then he could go for the grab and genjutsu.
Chances are he would never pull that off in a 1v1.

The only thing Sasuke is superior in is stamina. Itachi's Susano'o is stronger due to the spirit weapons, and he's superior in basically everything else aswell.

How could he hold back Tsukuyomi or Amaterasu? Sasuke countered those jutsus and everything else he used except Susanoo. And this was prior to getting his own MS.
Just because he wanted Sasuke to kill him doesn't mean he wasn't fighting seriously. He had to make it believable and he wanted Sasuke to develop and at his strongest.
