I think it was one of the other Kages, probably the First Raikage or Tsuchikage because they looked the strongest of them all. You're prolly didn't understand the story at all. The Uchiha and Senju clans were always the strongest, and there was no such a person who could even injure Hashirama, except Madara ofc.

Nope. Kakuzu was sent by his village to assassinate Hashirama, but he failed. The village punished him for his failure, so out of anger he stole a kinjutsu, killed the village elders, and became a missing-nin.

It's possible that he never fully recovered from the Madara fight(inspite of his healing), and his healing abilities may take up life force like Tsunade's, so he might have gotten weaker as he aged.(hence making him easier to kill). I doubt any of the kages were as strong as VOTE hashirama.
