I'll get right into it. This thread will be about Kamui and Kotoamatsukami; in my opinion, two of the strangest jutsu in the series! Now, let me start of with the two more common Mangekyō abilities. Susano'o, Amaterasu and Tsukiyomi. These three abilities are held by Madara (although we haven't seen him use Amaterasu or Tsukiyomi, we can assume he can. As Itachi said Susano'o can only be preform by an Uchiha who has awoken Tsukiyomi and Amaterasu in their Mangekyō.

Also, he wants to reflect his eye off the moon and cast the infinite Tsukiyomi. Therefor, we can assume he also has Amaterasu.
Interesting thing here is- all three people who have these abilities- also have siblings. The potential to gain the Eternal Mangekyō Sharingan.

Kamui and Kotoamatsukami are weilded by both Uchiha Shisui and Obito- both of which haven't been shown to have a sibling and, therefor, no chance to get the Eternal Mangekyō Sharingan. Because of this, they have unique and specials attributes that are unique to them (Obito's case is different, obviously.)
Remember the Oni used to we we're shown when Itachi was explaining how one acheives the Eternal Mangekyō Sharingan to Sasuke? I think in their cases, they only have one pair. One ability. Because of this, their eyes also never seem to lose their 'light'. The eyes are born eternal. Evidence of this is that Tobi has used Kamui countless times and never once has shown any evidence of deteriorating vision. In Shisui's case, both his eyes could Kotoamatsukami once a decade. So, twice every ten years. However, after using it, it was never stated he'd go blind.

So. Uchiha who are born with siblings are cursed to become insanely attached to them. Due to Uchiha being cursed to love. This is why the stone tablet said you must kill your brother. After losing the person they're attached to, the Mangekyō awakens. If the Uchiha in question has siblings and the potential to get the Eternal Mangekyō Sharingan, they will be granted Amaterasu and Tsukiyomi, thus giving them Susano'o. If the Uchiha in question is an only child and has no potential to achieve the Eternal Mangekyō Sharingan- or rather- is granted the Eternal Mangekyō Sharingan upon awaking the new eye, they will receive an ability exclusive to them and dependent on their nature. Example; Kamui complimented Obito's situation. It was the only way he could survive his scenario and his eye knew this and awoke in that moment to save him in that way. Kotoamatsukami complimented Shisui's dedication to the Leaf village. Giving him the ability to save it had he used it sooner.
