It doesn't spawn, it travels. It was shown against A. It's just that it travels so fast that normally it's off-panel and the next panel shows the target already hit with it. Seriously, A would not be able to dodge it otherwise.

If the flames were unextinguishable, then Nagato wouldn't be able to extinguish them. It's a hype, just like the hype about black flames being as hot as the sun. Obviously, if that was the case, the single use of Amaterasu anywhere on the planet would cause everyone and everything to burn.

Well yes but what i meant was that their un extinguishable when left alone for a time, yes i know they die down eventually But that still does not change the fact that if they spread they would leave a trail of black flame leading towards the target,

Hard to say... only way Itachi could "counter" Kamui is by using clones. And kakashi is smart enough to not get fooled like that. It could go either way really. I'm going to say itachi wins though because he has more than one destructive jutsu unlike Kakashi. It would be very close though.
