EMS Madara could just as well sit in his PS to not get pierced by Totsuka. Unlike Itachi, Madara has EMS and won't go blind. Unlike Itachi, Madara has a high amount of stamina.

He didnt react to the lightning though. He reacted to Sasuke's hand moving downwards to "fire" the lightning.
Similar to how someone can dodge a bullet by moving milliseconds before the trigger is pulled.

There's a difference between "I'm going to Shunshin you", and "Look at this! This is Kirin! With this I just direct the lightning at you. And then you're finished! Now, I will send up Chidori to begin taming lightning, and then form a lightning dragon who will THEN proceed to hit you at the speed of lightning".

He had enough time to put up Susanoo. Even if he didn't do it while Sasuke was talking, a decent amount of time passed between when Sasuke sent up Chidori and when the lightning dragon came down to hit Itachi.

That was most likely to showcase Susanoo. If Kishi would have showed it before Kirin struck, it wouldn't have been as good an introduction. Just like how he had the storm clouds form while Itachi and Sasuke were fighting, with Itachi not noticing them until the prep was more or less ready.

Itachi was blitzed by Kabuto, who sure as hell isn't as fast as lightning, which is faster than the speed of sound. And he was fighting him already, so he would have been watching himself for attacks. It wasn't a case of Kabuto sneaking up on him randomly while he was strolling around.

The logical conclusion is that Itachi put up Susanoo when he saw Sasuke sending up Chidori to form the lightning dragon.
