Er, no, he didn't. It was clearly implied that while Sasuke was busy boasting of his victory, Itachi prepared the Susanoo. If Sasuke had just performed Kirin without bothering to explain it, Itachi would have died.

Again, all you need to do is wait until Itachi gets tired and loses the Susanoo due to his low reserves. Itachi not only has low stamina, but MS, meaning that he would go blind if he uses it for too long (Sasuke had MS for something like 3 days before he started going blind).

Edo Hashirama could use Mokuton to avoid getting pierced by Totsuka (he's not Orochimaru. Even if he doesn't know that that's Totsuka, he won't just let Totsuka pierce him). Edo Madara could use Shinra Tensei to push it away. Naruto, Minato, A and Tobirama could just avoid getting pierced by Totsuka.
