Some people seem to not understand that speed isn't everything. Even though someone is quicker you can still anticipate their moves. A single block or hit from tsunade and it's over. If she blocks an attack she can easily catch him and throw him into the ground and pound his ass. as long as he hasn't opened several gates it's only a matter of time before she actually hits him. And if he opens his gates he will be an easy target afterwards. So many people here underestimate Tsunade She isn't as slow as she appears and she became Hokage for a reason. The only leaf ninja who could beat her I think would be Kakashi and Naruto.
Sure! If Tsunade manages to tag Gai, he's done for. But what makes you think that Tsunade even remotely stands a chance of coming into physical contact with Gai? Even looking past speed, Gai's taijutsu expertise alone enables him to effortlessly dodge Tsunade's sluggish, predictable attacks. And to anyone who thinks Creation Rebirth lasts longer than the gates, then you may want to revisit the manga. Gai has been fighting for a VERY LONG time switching the gates on and off at his own discretion. Tsunade does not have that luxury with Creation Rebirth. Gai can very well indeed "outlast" Tsunade.

Hmm, I wonder, Tsunade had Katsuyu attending to nearly everyone in the village and she healed the people to whom Katsuyu was attending. Now if there are roughly 16,000 Shinobi there are many more citizens of konoha, it doesn't seem as though shinobi comprise the majority of the population in fact as all other jobs must be performed beyond fighting and espionage. And even if Shinobi's did comprise the majority of the Konoha that's still nearly 20,000 people Tsunade healed, and prevented from dying. Yes a lot of people died but compared to the village's total population she still managed to save many lives. And it's not like healing requires only a little bit of chakra.

I'm assuming that an attack as fast as Morning Peacock could probably out damage Tsunade's healing. There is NO solid evidence that Tsunade could tank any of Gai's final gated attacks, none at all. Purely speculation...

I'm not arguing that Gai can outlast a master healer, of course not that's absurd (It also depends on your definition of outlasting). I'm referring to "The Gates" Vs "Byakugo" in terms of duration.

Gai wins with a direct hit from his susanoo-busting Hirudorah.
If she can dodge that, then she'd more likely than not outlast his gates with her byakugo, and if we consider the damage that gates do to his body, one hit on him and his battle capability is going to substantially drop. Also, Gai isn't going to be speedblitzing her, at best he's on par with Ei's V1 speed while in the upper gates and tsunade should be able to at least put up a guard against his hits.
So I'd give Gai the slight winning edge in this match.

Unlike the Magatama's, the damage that Morning Peacock inflicts, occurs in rapid succession. Gai's multiple punches with Morning Peacock seem fast enough to overcome her rapid regeneration. But again, that is speculation.

I just say that she can heal almost every damage on her organs
Gai has no regeneration he has speed but speed is not everything
She has katsuyu instant regenration and strength that is unsurpassed so i give this to tsunade
Even if gai is one of my favourites

Just imagine it... Gated Gai w/ Nunchucks knocking her around the battlefield like he's playing ping-pong. She just doesn't have the necessary speed, reaction time, or taijutsu expertise to deal with him. Tsunade ain't ready for the The Power Of Youth yet
