No one up to now knows what his real name is. His code name in the ANBU was Tenzou. Then when he was assigned to Team 12 by Tsunade, he was designated a different codename. That code name is Yamato.

Anyone claiming they know his real name is just a big fat liar as the manga does not state what his real name is. He is an enigma of sorts.
