Tsunade's regeneration has a limit.

If she could regenerate entire limbs then she would have grown back her lower half after she was cut in half.

Her Byakugou wasn't active , she was using it to heal the kages. And kishi didn't need any plot nudity unlike madara tsunade doesn't have any organic clothing

Just thought I would leave a nice aid for kids to understand, healing and regeneration are actually the same thing the body only heals its self when it creates new cells to fill the missing gaps. The wiki article also explains is much greater detail how humans heal by using regeneration. Macro****es in the wiki article is what is in control of regeneration, skip to that if you do not feel like reading the whole article.

If you mean about healing others, obviously Tsunade >> Hashi, but from Madara's reaction they are pretty much equal in the self healing compartment.
