Why itachi did not use izangi while fighting sasuke?

Because he want to leave his eyes to sasuke to get EMS that is the reason why he mentioned and explained about the EMS shit as he knows that sasuke would transplant them in future and even planned for it before hand if he ever go rogue against the leaf.

izanagi won't stop an illness, though itachi was'nt a power freak, if he wanted power or to survive, he can (but you know plot) he can just go to konoha (he knows how to bypass the barrier undetected and shit) and genjutsu tsunade to cure him, oro was already his biatch, so he can go get him some hashiroids before he later transplants sasuke's eyes, but ya know, he'll keep soloing and that won't be good for the mc's

I believe itachi could have set izanag for some days during or at the being of the match against sasuke,then let sasuke kill him in the process as the manga.as a result,sasuke will still unlock ms ,and finding out the truth.when the izanagi set period is finish,itach can still keep an eye on obito and shinobi world,preventing the moon eye project .in addition,itachi could have use shishu eye as his replacement eye .i know,he wanted to die ,but he also knew there were numerous unfinish issue to finish
