He had a terminal illness. From the way he profusely coughed up blood, it was definitely consumption. It seems that his lung collapsed from the stress MS produced on his body.

He got infected during his 'fight' with Orochimaru. Itachi thought he soloed Oro neg diff, but in the end Orochimaru survived and Itachi died like a fodder to his the disease.

Someone made a thread back in October entitled "The Disappearance of Itachi Uchiha." The theory made the detailed case that Itachi's symptoms were the physical manifestations of his emotional torture.

I am inclined to agree with it.

I too think his illness was caused by guilt due to the killing of his clan and the fact that he had to separate himself from Sasuke who he loved more than his own life. I think that was the conclusion Kishi wanted drawn from the fact that he never named the illness.
