Where would Tsunade be now if Jiraiya had decided to accept the role of Hokage? As he searched for Tsunade to become Hokage. And maybe, would have searched for her either way.

Would she be involved in the war, and - considering Jiraiya most probably wouldn't have died - how would he fit in the war?

Personally, if Jiraiya was Hokage at the time of the summit, he surely would have been named the "supreme leader".
