^ This is the only certain flashback that shows coughing with Obito referring it as a disease. But it is from the battle. It seems either like COPD or tuberculosis.

Before Itachi had a fight with Sasuke, Tobidara tried to kill Itachi and they both had a fight, which ultimately led to Tobidara's defeat, but Itachi was still severely injured/exhausted from that fight. And right after that the latter procceeded to fight Sasuke later on.

This happend to you ,since ur so familiar with it ?Rumor has it he and shisui engaged is sexual activities on a daily. Sometime during those events itachi contracted aids, shisui felt guilty and offed himself and itachi ran away from home due to guilt.
