The problem was that this jutsu has not been fully explored. And people are hypocritical and selective in what powers and strengths certain characters have.
For instance, if Toskuta blade really does GG no re everyone and that yama thing shield defends against everything, why isn't half the akatuski dead? or why wasn't Naruto a skewered 9 tails when Orochimaru tried stabbing him with his all piercing throat sword?
Tsunade's skill is hyped up, we don't know if she really could survive a decapitation. Though if people are content to assume the above weapon descriptions are true no matter what, then yes, Tsunade's skill would allow her to survive basically anything, since every cell destroyed is instantly healed.
However, that' isn't the case, Tsunade's regeneration would allow her to tank hits, but it's not instant if manga scans prove anything. I'd say it's a couple seconds behind. Enough to prevent normally fatal wounds (like getting burned alive or getting stabbed in the gut) But a pure decapitation? like one hit sort of thing? Not sure if she could survive that.

But the probability of anyone actually taking the time to cut off some one's head in battle is so high risk it's not worth it unless they have a specific ability to do so (Zabuzu for instance, he's sword literally lives for decapitations, and that's generally AFTER he's beaten them down, an execution move).
