when he looked at the spot and used amaterasu, the flames most likely landed on and slowly ate through the toad's throat. then i guess it got big enough for kisame and itachi to use it as a canal and enter the outside world

Itachi burned that toads mouth with ease that even Jiraiya praised. It by no means was a slow burn Amaterasu burns faster if the user is focusing on its target.
