Tsunade said that Lee's injuries in part 1 were really dangerous and it was needed a very risky cirugy. Guy is breacking into ash, broke all his bones, only has that little bit of chakra left and Tsunade is out of chakra. Unless Naruto does something OP to him, Guy will die
