I've actually only heard one of those insults, ever. Seriously, and that was number two (Chicchai ketsu no ana). It just means small ass hole, but it usually refers to a close-minded person.
Honestly? Now-a-days Japanese people don't use complex insults. Everything I hear is simply a noun-clause describing the stupid things they do tied with 'baka yarou' or, more commonly heard in Osaka, 'kono do-aho'.
