OP says this is prime Tsunade, so she doesn't have to waste chakra for her appearance technique and she's faster too. Her strength should be the same or greater taking into consideration her natural strength as well. She wins this with mid-high difficulty IMO.
Adult Sakura beats war arc Tsunade though with high difficulty and ~70 year old Tsunade ( current )with mid difficulty.

It would be a draw like Sakura vs Ino if they are both prime.
Tsunade have more battle experience than Sakura is a stupid argument, given Sakura intellegence feats over compensates for her lack of battle experience , this was literally stated during the chunnin exams written test. And this is how she was able to match sasori.

Also Sakuras jutsu execution is slightly faster than Tsunades, as she doesn't require handseals to release her byakugou and tsunade does.


In a canon fight, Sakura would probably pull out some genjutsu feat to win, as Kakashi predicted that's the only way to truely surpass Tsunade e

Prime Tsunade is like making up statistical bullshit about Prime Hiruzen, or any character who's prime we've not actually witnessed. Fanfiction.

That said, if your statement is correct. Sakura should win due to reaction superiority. Reaction is by far the greatest attribute in the speed category one can possess. Trumps movement speed honestly, unless your opponent is leagues faster than you, which Tsunade isn't.
