Tsunade's chakra reserves are huge. So if she enters Byakougou she doesn't need to see him he's going to try to Hirudora her... and she's gonna be back up and he's gonna go wtf? then try morning peacock. She'll get back up and he's going to be screwed.

Guy without Gates wouldn't be much of a problem vs Tsunade she wouldn't even need Mitotic Regeneration, she could just use regular medical jutsu.

Guy with his Gates would require her to use The Strength of 100 technique, to keep up with guys constant damage he will deal to her. Tsunade could further rip guys muscles with the chakra scapel jutsu, then she can break his bones with her massive strength.
Also Tsunade only needs to chop him in the back with Body Path Derrangement to make Guy lose control of his body and since he is no medical Nin he would have no chance of regaining control of it.
