I was just about to say Kakashi, when I saw that Kamui was restricted...

Honestly... Without Kamui he's barely low kage, and Tsunade is low to mid kage in my book.

Kakashi can't really do so much while Byakugō is active. None of his attacks will be able to finish her off... And while Tsunade will probably have trouble landing a hit cause of Kakashi's speed, I'm pretty sure she would land a hit sooner or later. And then it's pretty much game over.

So I see Tsunade taking it high diff.

He has never utilized such a strategy against serious opponents and I doubt he'd get the chance with Tsunade pressuring him at close range. If you try to use a speed argument, Hidan managed to fight evenly and pressure with Kakashi despite only having a 3.5 in speed.

All the conditions are in Tsunade's favor, but Kakashi still takes it, probably high difficulty, though, because of Tsunade's annoying regeneration and, even more so, Katsuyu.

Seriously though, base Kakashi is on the level of four immortal V2 jinchuuriki with Sharingan and Rinnegan.
That's like more than 4 times stronger than handless Orochimaru.

Tsunade doesn't compare.

Kakashi takes the field in intelligence, ninjutsu, taijutsu (not raw strength, though, obviously), genjutsu, speed...
Tsunade, on the other hand, has regeneration and a powerful summon - though she cannot summon even 1/10 of Katsuyu on her own.
I don't see Tsunade herself harming Kakashi in any way, but Katsuyu is dangerous. That's the only reason I believe Kakashi might be having high difficulties with winning and not mid or low.
