This is a hard one for me, tbh. I'm huge fan of both Fifth and Kakashi.
I think Tsunade would win just because of her kenjutsu.

She won't survive Kamui. Her taijutsu can be evaded, even P1 Kabuto was able to. Intelligence? Kakashi has a brain comparable to that of Shikimaru's.
Holy shit. And I thought the Tsunade wank wouldn't get any worse. KENJUTSU?!!!
When has Tsunade ever used swords? If it's a straight up kenjutsu match Kakashi destroys her.

A wood clone which Tsunade managed to one shot being stronger than Kakashi is baseless.

If Naruto didn't arrive he was going to use it on Kakazu.
In the war he was matched up against multiple opponents, it would be foolish to resort to his most powerful move.
Fights in the manga would be antilcmatic if Kakashi consumed every opponent with Kamui.
If Tsunade pushes Kakashi to his limit, he will resort to Kamui. It's as clear as day (unless he decides to resort to it earlier.

You still have no proof, just theories my freind, the only person he used it on was Deidara, and MAYBE Tobi, As for the rest, no where does it say that he was going to use it offensively, he may say hes going to use it but that doesn't mean he's going to use it offensively.

First is that Kakashi himself said to Naruto how he would've been forced to use the Mangekyou to sort things; when they were talking later on after the fight. He wouldn't claim he could do it, if he couldn't in actuality. He seemed to be sure he'd end up at the hospital, but he would've managed to take Kakuzu out with his MS.
All in all, Kakuzu was still holding onto Kakashi and was charging his attack. The time window in which Naruto and Yamato intervened with their combo, Kakashi could've used Kamui. The above ^ feat against Sasuke proves he's capable of such a thing. And since Kakuzu was standing right there in front of him, he would've had a clear shot at his head/whole body

What proof do you need more?
He almost got Deidara with an unmastered kamui while the target was flying and escaping at very long range and you think that he can't use kamui on a late Tsunade at middle range like he was going to do vs Tobi?
Databook is clear with kamui.

kamui is his only hope, and no defense? all you have to do is make kakashi moved around (not in a linear way) and he'll lose focus or he'll try and keep focus straining his body and chakra.
anyway kakashi only uses it for defensive purposes now, since he knows the toll it takes on him i guess
