this is my made up story on how Tsunade obtained Slug Sage Mode.
After Tsunade was sliced in half by Madara Uchiha, He immediatley fled the field leaving Tsuande to die. Madara left a huge scar on the battlefield. This signaled a portal to appear right by Tsunade as if it was her last hope of saving herself from the brink of death.
Tsunade could barely move at all because of her serious wounds but she only needed to reach her arm out so she could touch the portal. Any interaction with the portal it would whisk Tsunade away to an unknown dimension. She thought about all her loved ones then she used all her strength and might to reach for the portal and she succeeded. Everything was a blur to Tsunade then all of a sudden she appeared in an unknown world. Tsunade used Kuchiyosi No Jutsu to summon Katsuyu. Katsuyu immediately rejoined Tsunade's body back together and restored her health. Her vision was back to normal, then all of a sudden she immediately had a feeling of trepidation because she never had witnessed a world like this. Tsunade started to stroll across this unknown world and was being very cautious of her surroundings. She came across this ghostly town with no one there except herself then all of a sudden this bang shook the entire place causing a earthquake so her only safe option of escaping the earthquake was to hide in a building in the town. After the earthquake had ended Tsunade escaped unharmed. Then suddenly she noticed these two people fighting so she kept her distance. This Super Namekian called Lord Slug dismantled the earths mightiest warrior who's name was Goku. Lord Slug tossed Goku arround like he was nothing to him. Tsunade couldn't believe what she was witnessing. Then a memory triggered in her mind telling her that her friends need help. Tsunade came up with this amazing plan.
This would be the only way to help her friends so she summoned a baby Katsuyu and told her to hide in Lord Slug so Katsuyu could absorb Slug's energy. Tsunade was amazed by Lord Slug's power so she thought if she could absorb his power she would become unstoppable. After Katsuyu absorbed his power she came back to Tsunade and gave her all that pure energy. Tsunade's Chakra reached unmanageable heights and she was a force to be reckoned with. She transformed into Slug Sage Mode and she was ready to travel back to her own world with a burning desire to rid the field of Madara and save her friends from the chaos that has erupted in the Ninja war. She went back to the place where the portal dropped her off and there was no portal there to whisk her back. Tsunade thought all hope was lost so she started to cry and scream, then all of a sudden the portal appeared and she could not express her delight so with Lord Slug's energy flowing through her veins Tsunade went through the portal to go back through the dimension to settle unfinished business.
