Tsunade stepped down because she didn't want to be hokage .
Gai or kakashi wasn't really the next logical choice , it was Sakura, because she had the power of the previous hokage and the intellect of a hokage .
Sakura didn't want to be hokage though
Kakashi only got it because Tsunade declared him hokage earlier
