Itachi is at his healthiest, he has no illness or health deficiency and is as healthy as ever. Kisame too is at his strongest and has Samehada with him. Tobi has his current abilities, Rinnegan, Gedo Mazo and here's how the fight is outlined:

Location: ocean

Starting Distance: 100 METERS

Condition: All healthy and at max stamina

restrictions: Katoamatsukami

scenario: Tobi summons his Six Paths and has them go in version 2 Jinchuriki mode and has the Gedo Mazo summoned, Itachi activates his Susanno and wields his legendary weapons alongside, Kisame fuses with Samehada and the fight starts from there. So, who wins?

Kisame and Itachi are fighting in the OCEAN, And Kisame fuses with Samehada right away. Kisame under that circumstance alone can cause major damage to Tobi and his paths, adding a healthy Itachi in the equation and either team wins with major difficulty.

It can go either way, whoever wins will win with extreme difficulty.

BTW, Kage level Fire techniques, Meteors, EMS, Wood release, Rinnegan, and Kage level taijutsu and speed are equally as impressive imo
