you are over thinking this. he was just to badly injury during the fight fell in to the water and swept down river and no body was found so everyone assumed he deid, the easy answer is sometime the best answer.

i think ems allows the user to use izanagi without going blind in fact im almost sure u can tgo blind with EMS thats the point lol

Madara already stated izuna died, and that izuna didnt die after he gave his eyes to madara he died in the war cause he was blined
