What you do is after building up a fairly large ammount of chakra you release it all while forming a thick and powerful fire in the shape of a large dragon head. Usually this attack forms two blasts using the chakra you gathered up. It could be more depending on how much chakra you used.

Now when you exhale all of the chakra form the head of a dragon while compressing all of the fire to have maximum effect.
