yeah and shodaime is still referred to as a hokage... i know it's usually preceded with "1st"

he might have referred to him as mizukage madara possibly like from pictures in books etc

it's an open story... open to interpretation

I thought it seemed like the Mizukage was actually the old man who gave the woman the kage hat. She calls him sir meaning that she respects him and is most likely below him. I think shes just going in his place because hes too old. But its nice to know you all think an old man is hot.

It was also noted that she was extremely kind, so it could just be to make others not seem so mannerful around her just because she is a kage. That guy could also be a village elder.
Plus, she says "In the name of Mizukage" or "because I am Mizukage". One of those two. Who knows. But I think she is Mizukage.

by the way hokage means the kage of konoha -__- so your basically saying the mizukage is like tsunade,minato,sarutobi,shodaime,nidaime and etc.
