So I have written this theory on paper a couple of times now...
Basicly the 10tails was composed of 2 parts. The eye and the Body.
In this theory I decide that the Gedo Mazo statue is the 10tails former body. As you can see it has been stripped of its tails and it wears a blindfold because it lacks its eye.
When The Sage of the 6 paths (God i hate Kishi for not giving us a shorter name)
sealed the 10tails he learned its powers and recieved the "body" and the Rinnegan.
just like Naruto awakend the "body" from controlling the 9tails chakra so did the Sage.
Before the Sage of the 6 paths died he wanted to make it easier for the world/his descendants to protect the world from the 10tails. Since it was too powerful he divided the chakra using Izanagi. (God i have written this too many times). Izanagi only closes one eye. So he could have used Izanagi two times.
1.He divided the 10tails doujutsu. This was split into 3 doujutsus, The Byakugan, Sharingan and Rinnegan. The Byakugan was given to someone close to him who became the Hyuuga ancestor. This would mean Hyuuga is not related to Uchiha but the eyes still connected which kinda makes Kakashis "rumour" in part one a bit true.
Since his first son wished for power he granted him the Sharingan.
In a picture off him you can see that his eyes do not resemble eyes but a sealing jutsu. He most likely just had a normal sharingan and the power hungry eye led him to attack his brother since that is what awakens the sharingans power. (Since the eyes comes from the 10tails they are evil the Uchiha are tainted by the 10tails evil, which gives more meanin to what the 9tails said to Sasuke)
2. He split the Body creating the 9 demons. He sealed one of them inside his second son, giving him "the body".
Madara has only said the body and eyes were "given" not that they came from genes.
So the Sage sealed the eyes and he sealed the body inside of his sons.
After he had disposed the Byakugan and Sharingan and his own Rinnegan had been blinded he needed to dispose the last Rinnegan. He wanted to hide it and asked his first son to do this. But as he was only interested in power he took it for himself and it was kept somewhere within the Uchiha clan. Later Madara would find this Rinnegan and give it to Nagato whom later awakend it by the loss of his family.
An important question here is why did Konan and the Sage never destroy the last Rinnegan in the first place? Konan knew Madara would come after it but she still didnt destory it. This is because neither she or the Sage could.
The 10tails chakra is immortal. Its body has survived since the Sages time and if the host would die the Bijuu would roam free as what happend with the 3tails after (presumably) Madara killed Yagura.
So there must always be one remaining eye of the 3 doujutsus.
In order for Madara to awaken the 10tails he would need to collect all the eyes including the Byakugan thats why he has a third hole in his mask. But I dont think he has to actually implant it into his forhead...
In the latest chapter I think that Nagato took the 9tails from Naruto by using the Human path. He then recieved both the body and the eyes and therefore became complete as said in the manga.
I think that the 10tails was created by human sins and it cant be destroyed until world peace is achived when that happens the eyes will also disapear.
(The 10tails eye has the combination of "pure white", ripple pattern and tomes.)

, but about the Nagato taken the 9 Tails from Naruto could be unlikely. Naruto is the main character and needs the power of the 9 Tails to stop Sasuke. Without that i don't think he could be strong enough to beat him. If I'm not mistaken Sasuke does have the EMS and maybe the Rinnegan.
