Susanoo is the Disease

In the databook Susanoo is stated to slowly kill the user, or more specifically it eats away at their life-force. This is stated to be the cost of Susanoo.

Sasuke also mentioned how every cell in his body felt like it was on fire while using Susanoo, establishing that it attacks the user's body... like a disease. He also noted how he hadn't perfected Susanoo yet and wondered what the strain was like for Itachi's body whom did.

Sasuke, like Itachi, also staggers and coughs up blood. This is a sign of internal damage, that being done by Susanoo.

People have also speculated that Itachi's disease was tuberculosis, which is a lung disease. For those who may not know, in myth Tsukiyomi and Amaterasu were born from Izanagi's eyes while Susanoo was born from his nose. So is it a coincidence that Susanoo may damage the lungs (i.e. body) while Tsukiyomi and Amaterasu damages the eyes?

Susanoo stemming from the body could also explain why Itachi and Madara were able to use it while blind, with the MS just being needed to unlock it.

In summary: Susanoo IS the disease.

Possible but what about madara

I was thinking this too. But maybe the reason it didn't "infect" Madara so to speak was because of his brother's eyes. Maybe they acted like antibodies to the cellular decay caused by Susanoo. At one pont though Madara was suffering from blindness so can't say he was completely immune at the time. It only stopped affecting him after he obtained EMS much like it did for Sasuke.

A plausible theory but in reality susano is the embodiment of a users soul, remember if senju's can externally portray their power due to them being yang uchihas being yin internally exert it which explains the high level jutsus they can perform along with susano which is a spirit poured outward due to susano not just being a chakra embodiment it is a spiritual embodiment notice how each susano looks like their caster you can look at kakashi's susano for example who had the same slash mark as kakashi did. Now for the reason madara felt no pain is not because of large chakra reserves but also there's a thing called adaptation that humans have, it allows us to get used to something and adapt to it. Think about it if any of you know anything about certain diseases is that when we're new to it we suffer pains, symptoms, etc from it but gradually we begin to build up an immunity if you will towards that disease basically madara has adapted and adjusted to the pain, susano is merely the caster's spirit poured outwards which may be why it hurts so much, but it's only the chakra of that spirit think of it as susano as a medium between the spirot and reality, as spiritual chakra.
