Seeing how morals and ethics are given more priority in Naruto than the gender, I think the story would be about the same as it is right now. Except of course the series would have lot more sexual tension and pairing wars would be rampant

Thing is, her entire backstory and plot primary revolves around-

1. Liking/loving Salsa

2. Hating Noodle and later learning to like/love him as well.

Salsa being a female would force Sakura to

1. Disappear/ be on another team or

2. Have her entire character renewed or

3. Make Sakura a male.
