So! Shisui's only chance at using KA is the instant he first steps out of the coffin. Before Kabuto binds him with hand seals and then puts a talisman in his skull.
But yeah I'm sure that's plenty of time for Shisui to get out of his confusion at being resurrected, recognize Edo Tensei (assuming he even knows it), and then cast Kotoamatsukami on Kabuto. All before Kabuto does 1 hand seal.

When Itachi arrived to where Kabuto was, he was planning on getting Kabuto caught in his Tsukiyomi and forcing him to tell him the hand seals to release Edo Tensei...but Kabuto shut off his vision completely and had to rely on his sensory abilities. Shisui would have the same problem, he wouldn't be able to get Kabuto caught in Kotoamatsukami and he has no non-ocular genjutsu feats...he'd actually be worse in this situation.

-It was never used on Ao. Ao is the byakugan user who noticed it was being used on Mifune. We don't know if Danzo showed Mifune his eye beforehand. But we do know that while the KA was active, Danzo did continuously look in Mifune's direction.

-Danzo removed his bandage from his head when he stated that he wanted to KA Tobi.

It requires eye contact. Or at the very least for the caster to look at the eyes of the opponent.
