Itachi is better in Genjutsu. Sasuke is better with Amaterasu, with his Blaze release. Sasuke also has TWO elements, Fire and Lightning. I will almost say that Sasuke is better than Itachi in Ninjutsu, because of that.

However, Itachi is smarter, and faster(?)

Yes, Itachi is going to win, but with extreme effort i'd say. What do you say?

Itachi hands down, master of Genjutsu to weaken Sasuke. Itachi's susano'o is supreme compared to Sasuke's. Amaterasu is the only thing I'd give Sasuke + thr fact that he has Lightning+ Fire and to counter this elements Itachi brings up his 1. Genius mind 2. Speed 3. Experienced ninjutsu, overall a healthy Itachi wins. Mid-high diff.
